Buried Alive is bad, flaying sounds awful.  The worst though would be to be stuck in a rocky crevace out in the wilderness, immobile until you die of thirst.

With the exception of drowning, the rest of these are murders or suicides so I'd say they are all 10s.

Screaming to death? I suppose torture (undoubtedly the worse way to die) could make one die in that fashion....

How about some quicker/happier/more pleasant/honorable ways to die...

1: During orgasm 
2: In your sleep
3: Doing something you love doing (such as mountain climbing, etc)
4: Rescuing another person
5: Fighting for your country
6: Defending yourself during an attack
7: During childbirth
8: Out of the blue, e.g. a rogue meteorite smashes into your head at 12,000mph.  ;D
9: While attempting a world record.

or

10: Do something so incredibly stupid you get a Darwin Award.  ;D ;D

Working on those two as I speak.  Too many years spent learning how to cook when I was poor.  Too much disposable income now. 

And I'm far enough along on #13 (I cracked open a bottle of 70s-era vintage port this evening, by Dobbs this stuff is fantastic!) that I'm willing to contemplate my own mortality tonight.

As a "10", any death that involves lack of oxygen and buildup of CO2.  It's not the lack of oxygen that causes terror, it's the buildup of CO2 that causes the panic reaction in mammals.  As evidence, people who die of carbon _mon_oxide poisoning are happy.  People can walk into a room full of pure nitrogen and helium and die of asphyxia*thud* - before they know they're in a low-oxygen environment.  People who drown and _inhale_ the water as soon as they realize they're toast tend to die quickly.  People who hold their breath and fight it get all the panic of CO2 buildup, and all the not-fun of lungs full of bloody froth.)

As a "0" ("If I could choose any way to go out...")  If I could choose any way to go, I'd like to "Ride the Bomb", like Major TJ. "King" Kong (Slim Pickens) in "Dr. Strangelove, or How I Stopped Worrying and Learned to Love The Bomb".  Skydiving is fun.  Skydiving on a nuke would be funner.  Plus, I'd get to see the inside of a B-2 and nobody would have to lose his clearance :)

I think the impending fear of death might also have something to do with it...

If someone told you they were going to gas you with Carbon Monoxide, you wouldn't just sit in a flower lotus position, close your eyes and say, 'relax, this will be over soon, think of a calm blue ocean.'
