O.k. I know you guys can help me out here.
I'm currently filling in a job application form for a job with the police. On the form they want to know how many days off sick I've had in the past three years, including dates.
Since I can't recall that information accurately and asking about it at work is liable to led to awkward questions do you think it would be o.k. to say something like
"In the past three years I have had occasional days off sick. There has been no significant amount of time taken off work for sick leave. The exact dates can be produced at a later stage if necessary."
Should I mention in this section that I took maternity leave in early 2002?
Thanks for any advice guys. I'd normally ask my hubby but he's incommunicado in Washington at the moment.

Thanks to everyone for the good wishes. I posted the application form off today. If its like any of my other job applications that'll be the last I see of it. >:(

Never, ever mention maternity leave on an application, resume, or at interview. It's illegal for them to ask it because it can result in hiring bias.
