How about the one from Dharma and Greg? The show was funny for a while but the theme song made me want to throw a brick into the TV.

I may be in the minority, but "The Brady Bunch" theme, especially when the poofter guys sang it that first season.
Also, "Blossom" makes me cringe.

Another word stolen from the Bob And Tom Show and "Ian St. Ian", the residential British character. If you don't know by now, Bob and Tom are two DJ who host a nationally syndicated radio program from right here in Indy.

One that comes to my mind is the kazoo theme of "The Brady Bunch Variety Hour". This was a version of the "Brady Bunch" theme played on kazoos. Later on in the theme, it becomes played by band music and is a thousand times better.

I can't think of a TV theme song that annoys me, I don't watch any current shows so I guess I wouldn't know their songs, but as for the old theme songs, I pretty much like them all! Especially Gilligan's Island. :)

My wife came up with an old parody fragment for that once..."Charles in charge, of our.....and our....." Add slang words for male and female genitalia, and you have it.
There was something about the "Battlestar Galactica" theme that made it a good parody target.

Recently, I watched the show from start to finish...and the theme song sounded too "Britney Like". :P
And any from today's cartoons on Nickelodeon like "Fairly Oddparents", "M.L.A.A.T.R.", and "Rugrats".
I know that Mark Mothersbaugh (sp) from Devo does the theme for "Rugrats" ,but it sounds lame.
